  • What should I expect during my first appointment?
    Expect a comprehensive evaluation by your provider to include discussion of your current symptoms, goals, limiting factors, and past medical history along with objective measures including strength and mobility testing, gait analysis, lifting mechanics, etc.
  • How long are appointments?
    Each appointment will last approximately 60 minutes unless otherwise specified.
  • What should I wear to my appointment?
    Wear clothing that is comfortable and easy to move around in. Athletic attire such as t-shirts, shorts, leggings, etc. is recommended. Be sure to wear athletic shoes as well. Your clothing should also allow your provider to easily access your symptomatic area (i.e. if you are coming in for shoulder pain, wear a t-shirt or a tank top). Feel free to change into appropriate attire upon arrival if needed.
  • Can I utilize online services even if I don't live in Washington State?
    Yes! You may use our online services, personal & wellness training, nutrition coaching, and top surgery training program, even if you do not live in Washington State. Physical therapy is only available for Washington State residents.
  • My symptoms are gone. What should I do next?
    While your symptoms may have improved, this should not be the end of your treatment plan. The goal is to prevent your symptoms from returning with progressive strength/mobility training. This might mean you transition from physical therapy to our personal training program.
  • How many sessions will it take to recover from my injury?
    All injuries are different and require varying treatment lengths. However, one physical therapy appointment per week for 4-6 weeks is the typical amount of time it takes to see improvements in an injury.
  • What payment methods do you accept?
    We accept all major credit and debit cards, Apple Pay, Google Pay, Tap-to-Pay, cash, and checks. Card info can be saved in your online account to make it easier to pay after your appointment. Invoices can be viewed from your online account.
  • How do I schedule as a new patient/client?
    Start by clicking the 'self-register' button in the header. This will prompt you to create an account and enter your contact information. After creating your account, you may book yourself for the service of your choice. You will be sent paperwork through your online account that must be completed prior to your first appointment.
  • How long will it take me to get in?
    Our goal is to get you scheduled within a week of when you want to be seen.
  • Do you accept MVA claims?
    Yes, we do accept Personal Injury Protection (PIP) claims following motor vehicle accidents.
  • How do I use my PIP insurance for physical therapy after a car accident?
    Step 1. See your primary care provider and have them fill out a "report of accident" form. Submit this form to your auto insurance company. Step 2. Get assigned a case manager. Step 3. After your visit at Evergreen Wellness Co. submit your superbill(s) to your case manager for reimbursement. Things to keep in mind: - Treatment must be performed within three years from the date of injury. -You can only be treated for injuries sustained from the accident. -Ultimately, it is up to the insurance company to determine if your treatment is "medically necessary" for your injuries. Please rest assured we will do our best to fight back if you receive any denials.
  • How do I submit a superbill to my insurance?
    Step 1. Figure out your out-of-network benefits for physical therapy. Use the form below to ensure you receive all of the information you need. Step 2. Determine if your insurance plan requires authorization or a referral prior to beginning treatment. While you do not need a referral to be seen at Evergreen Wellness Co., your insurance may require one for you to get reimbursed. Step 3. Log into your Evergreen Wellness Co. patient portal after your appointment where you can view/download your superbills. Your superbills have all of the information you need including diagnosis/treatment codes, service dates, provider license numbers, payment totals, etc. that your insurance will require. Step 4. Log into your insurance's patient portal and navigate to the "submit a claim" page. Here you will be able to upload your superbills. *Evergreen Wellness Co. provides you with a superbill at the end of each appointment. However, it is your responsibility to submit claims to your insurance. We will not submit insurance claims for you! We also cannot guarantee that your insurance will reimburse you, but we will do our best to work with you.
  • Do I need a doctor's referral?
    You do not need a doctor's referral to be seen. Washington is a direct access state, meaning physical therapists can legally see any person walking in off of the street. Insurance is typically the limiting factor in being seen without a referral. Cash pay eliminates this barrier.
  • Do you accept Medicare/Medicaid?
    We do not accept Medicare/Medicaid insurance plans.
  • Do you accept my insurance?
    We do not accept any insurance plans at this time. However, we provide a superbill after each visit that you may submit to your insurance for reimbursement. We accept all major credit cards and FSA/HSA cards.
